Blue Sage Tax & Accounting

Accounting for Healthcare Organizations

Healthcare organizations combine complex payroll, third-party reimbursement, and often a mix of taxable and exempt activity. Revenue arrives on payer timelines rather than invoice terms, which makes cash flow reporting as important as the tax return. Blue Sage handles the accounting, the entity structure, and the filings across every state you operate in.

What we handle

  • Monthly accounting with payer-aware revenue reporting
  • Accounts receivable and reimbursement timing analysis
  • Multi-entity and management company structures
  • Payroll accounting for large clinical and support staff
  • Contractor classification review for clinical staff
  • Multi-state registration where services cross state lines

Who this is for

Clinics, medical groups, home care and home health agencies, behavioural health providers, diagnostic services, and healthcare management companies.

The recurring problem in this industry is the gap between billed revenue and collected revenue. Books that recognise revenue at billing without allowing for payer adjustments overstate profit, sometimes considerably, and the tax return follows the books.

Deadlines and rules worth knowing

Key points worth knowing.
Item What applies
Revenue recognition Should reflect expected reimbursement, not gross charges
Receivables Ageing by payer, not just by date, is what reveals a collection problem
Entity structure Management company structures are common and require consistent intercompany pricing
Worker classification Clinical staff engaged as contractors draw attention from both tax and labour agencies
Multi-state Services delivered across state lines can create registration, withholding, and filing obligations

How we work

  1. Review what exists

We read last year’s return and whatever records you have. Most of what needs fixing is visible quickly.

  2. Fixed quote in writing

Scope and price before anything starts, counting entities, states, and any cleanup needed.

  3. File and maintain

Returns prepared and filed, notices answered, and a projection before year end.

Questions

Why does my profit look higher than my bank account?

Usually because revenue is being recognised at billed charges rather than expected reimbursement. If payers routinely pay a fraction of billed amounts, recognising the full charge overstates income. Correcting the revenue treatment brings the books closer to reality and changes the taxable figure.

Can clinical staff be engaged as independent contractors?

Sometimes, but it depends on the actual working relationship rather than the contract label u2014 who sets the schedule, who directs the work, and whether the person works exclusively for you. Healthcare is an area where both tax and labour agencies scrutinise classification, so it is worth reviewing before it is challenged.
